Affinia Health Network is a Clinically Integrated Network (CIN), bringing together a health network of over 1,400 physicians, four hospitals, and other professional health care professionals, working together, using proven protocols and measures, to improve patient care, decrease health care costs, and raise value.
The physician-led Affinia Health Network includes West Michigan physicians at Mercy Health and Mercy Health Physician Partners in Grand Rapids, Muskegon, and the lakeshore; the former Lakeshore Health Network and Muskegon Area Medical Association; and the former CarePath Partners in Grand Rapids.
The new organization will contract with commercial insurance payers, employers and other partners to provide high-quality, cost-effective care. The purpose of the CIN is to allow hospital-employed and independent physicians to better work together to provide seamless, coordinated care to patient populations, while improving quality and reducing cost and waste in the current system.
The CIN is overseen by a regional board of 13 individuals, including eight physicians.
